Qualifications

• Active New York State HHA certification required
• Minimum of one year of HHA experience
• Must be authorized to work in the United States
• Valid driver’s license preferred
• Reliable transportation strongly preferred

Required Health Documentation
Candidates must provide the required onboarding health documentation, including:

• Physical examination completed within the past 12 months
• PPD or QuantiFERON test completed within the past 12 months, or chest X-ray completed within the past 5 years
• MMR documentation, as required by applicable onboarding requirements
