Affirmed Home Care is seeking a compassionate, reliable, and experienced Certified Home Health Aide (HHA) for an immediate per diem hospice care assignment in Port Washington. 

This opportunity is ideal for an experienced caregiver who is passionate about providing comfort, dignity, and personalized support to clients in their homes.

Available Hospice Assignment Location: Port Washington, NY 

• Schedule: Friday, Saturday & Sunday 

• Shift: 8:00 AM – 8:00 PM (12-hour day shifts) 

• Pay Rate: $19.65–$20.00 per hour 

• Employment Type: Per Diem 

• Experience: Prior hospice care experience required

Key Responsibilities 

• Assist with personal care and activities of daily living 

• Provide compassionate support and comfort to a client receiving hospice care 

• Maintain a safe, clean, and comfortable home environment 

• Observe and report changes in the client’s condition to the appropriate care team 

• Follow the established care plan and agency guidelines 

• Promote client dignity, safety, and emotional well-being 

Qualifications 

• Active New York State Home Health Aide (HHA) certification 

• Minimum of 1 year of professional HHA experience 

• Previous hospice care experience required 

• Compassionate, dependable, patient, and professional 

• Strong communication and interpersonal skills 

• Authorized to work in the United States 

• Reliable transportation strongly preferred 

• Valid driver’s license preferred 

Required Health Documentation 

To help expedite the onboarding process, candidates should be prepared to provide: 

• Physical examination completed within the past 12 months 

• PPD or QuantiFERON test completed within the past 12 months, or qualifying chest X-ray documentation completed within the past 5 years 

• MMR immunization documentation, as required by applicable onboarding requirements
