Affirmed Home Care is seeking a compassionate and experienced Licensed Practical Nurse (LPN) to provide skilled nursing care for a pediatric patient in Freehold, NJ.

⭐ Pediatric Case Details

Freehold, NJ

• Pediatric patient with Autism Spectrum Disorder (ASD) and ADHD
• Skilled nursing needs include:
  • G-tube care and feedings
  • Compression vest treatments
  • PRN oxygen administration
  • Chest port monitoring and care
• Requires vigilant assessment, monitoring, and coordination with family and healthcare providers

⭐ What You’ll Do

• Provide 1:1 pediatric nursing care in the home setting
• Administer medications and treatments as ordered
• Monitor and manage G-tube feedings and care
• Assess respiratory status and provide PRN oxygen support
• Assist with airway clearance therapies including compression vest treatments
• Monitor for seizure activity and implement seizure precautions
• Assess for changes in condition and communicate findings appropriately
• Maintain accurate clinical documentation
• Collaborate with family members, physicians, and the pediatric clinical team
• Support the patient’s safety, comfort, and overall quality of life

⭐ Pay Rate

LPN: Up to $40/hr

⭐ What You Bring

• Active New Jersey LPN License
• Experience with G-tube care preferred
• Pediatric home care experience a plus
• Current BLS Certification
• Authorized to work in the United States
• Reliable transportation
• Physical exam within the last 12 months
• PPD or QuantiFERON within 1 year (or chest X-ray within 5 years)
• Proof of flu vaccine or approved exemption
