Affirmed Home Care is seeking an experienced Pediatric Licensed Practical Nurse (LPN) to provide one-on-one care for a sweet 2-year-old child in Greenwich, CT.

This low-acuity pediatric home care case offers the opportunity to work closely with a wonderful family while making a meaningful difference in a child’s life. Our client is making incredible progress following successful brain surgery, reaching new developmental milestones, beginning to enjoy solid foods, and thriving with the support of a loving and engaged family.

The case includes tracheostomy care and ongoing skilled nursing support in a warm, welcoming home environment where you can focus entirely on one patient and provide truly personalized care.

Responsibilities

As an Affirmed Pediatric LPN, you will provide skilled nursing care in the comfort of your patient’s home while becoming an important part of their healthcare journey.

Qualifications 

• Active Connecticut LPN license
• Current BLS certification
• Minimum 1 year of pediatric nursing experience in home care or acute preferred
• Tracheostomy experience 
• Eligible to work in the United States
• Valid driver’s license and reliable transportation

Pre-Employment Requirements

• Physical exam completed within the past 12 months
• PPD or QuantiFERON completed within the past year, or chest X-ray completed within the past 5 years
